  • Insurance and Payment: We accept all major insurances, but it is the patient's responsibility to confirm that our provider is in-network with their insurance plan. All treatment costs are the patient’s responsibility.

  • Appointment Format: Currently, all appointments are conducted virtually to best accommodate your needs safely and conveniently. We are planning to offer in-person visits in the future and will keep our patients updated on this option.

  • Emergency/Crisis Care: Mindful Clarity does not provide emergency or crisis care. If you or someone you know is experiencing suicidal thoughts, harmful thoughts, or a mental health crisis, please call 911 or visit the nearest emergency department immediately.

  • Refund Policy: We operate a no refund policy. Once services are rendered, charges are non-refundable.

  • Insurance and Financial Responsibility: We require a debit/credit card to be on file. While we do our best to verify insurance coverage, patients are ultimately responsible for any balances related to services rendered. To be eligible for FMLA form completion, patients must have been seen by one of our providers for a minimum of six months.
